When Data is added to a SDE Geodatabase it is automatically checked and repaired if needed.  If you think something happened to your database your best bet might be to create a new GDB has load your data into it.  Given you are running 9.3 you not going to be bale to get much support given how long it has been deprecated. ... I would consider using Survey123 for the actual inspection form.  You can still use Collector for all your mapping needs and have it directly linked to Survey123.  I did something similar with Fire Hydrants.  You could navigate to the point in collector and via custom popup create a shortcut to open the desired form.  There is also a methodology to collect the survey results directly to a related table. ... Brandon,      Survey123 collects and stores it information in a Feature class that has an attribute table with the rows and columns you need.  These columns are created by structure of your form and a new row is added for each survey completed.  I do not believe you can expose the table directly in Survey123 but you should be able to configure the a survey with the required question to produce the required data on the backend.  An alternate solution if you absolutely need a gridded layout for data entry would be to collect the data on the mobile device in a spread sheet or database and then join or relate the table to your spatial data after the fact.
