Thanks @DJerbCO , The form is really helpful, and I can reproduce the issue. I found the real cause is that when you press the "Enter" key on the input question, the map question accidentally triggers the "create new polygon" mode which clears the previous polygon. Sorry for the inconvenience, and we will address the issue in the upcoming Survey123 update, and I will let you know when the issue is fixed.  The "Enter" key should not trigger the map "create new polygon" whatsoever, but in case you press "Enter" to trigger the calculation, the web app can automatically trigger calculations when the input is changed, so the "Enter" key is not needed for the input.  Thanks, Ruth ... The survey description is currently only supported in the web app. The survey description published in the web designer will become the hint of the note question called generated_note_form_title. This generated_note question is hidden by default in the Connect and Field App. If you want to see the survey description in the Connect and Field App, please copy the hint to a note question. Hi @ArmstKP , The issue has been fixed in the 3.17 hotfix1 (currently available on prod), so the following 3 methods should fix the issue: Append ?version=latest to the survey URL or close the version lock In the website, fix the thank you screen message and republish the survey Republish the survey using 3.17 Connect We currently only fixed the issue in the 3.17 version, and we will try to fix the achieved 3.16 version in the next update (3.17 March update), then the survey with 3.16 version lock will work directly.
