I am working on something similar that might be a work around for you. I have a Survey123 form that uses a published feature service as a source for data and the database to store results of the form submittal (SQL Server backend). The feature service is shared via AGOL. I have enabled the Inbox in Survey123 (requires disabling the Sent box). The field user, after starting the Survey123 field app on the tablet opens the form and clicks "Refresh" to load the Inbox. What the user sees is a list of the inspections to be completed. The list view shows the structure ID only for items not inspected and map view shows the location of each structure to be inspected. After the inspection is completed, the date is added to the list view. Since I publish the feature service weekly, this works. For your use this may require more frequent publishing and manipulation of the structures to be included in the list. I am guessing that when published again during the week, the dates of inspections will be removed. If before publishing, I remove the structures inspected, there would be no chance of inspecting the same structure twice in the same week. I would be interested to see suggestions to improve/streamline this process. Work Force has potential but I have not used it yet.

A little more information will help with an answer. My experience suggests that a file is not where it is expected to be or some default setting has changed a path expected by the web app. From where does the "Address" information come - list from the choices tab in XLSForm spreadsheet? external CSV file? If from CSV, is the path correct now that it has been published - is the published form looking for the CSV file from a default folder on the server?

Due to the big update DougBrowning mentions, the solution mentioned will probably not work when you upgrade Survey123 to version 3.13.xxx. Before you update to the current release, read the link from DougBrowning so you know how to use the new HTML formatting codes

I have a slightly different approach that may not be helpful as it does not use Pulldata request. I created a select_one field with the name MONITOR that reads a short list from the choices tab with the list_name monitor and name choices of 1YR, 3YR, 5YR, and 10YR I next have a calculation row with the name nextcalc that has the formula if(${MONITOR}='1YR',date(decimal-date-time(now())+365),if(${MONITOR}='3YR',date(decimal-date-time(now())+1095),if(${MONITOR}='5YR',date(decimal-date-time(now())+1826),date(decimal-date-time(now())+3652)))) I finally have a text field named NEXTINPS to display the Next Inspection Date that is ReadOnly with the formula format-date(${nextcalc}, '%b %d, %Y') The Previous Inspection Date shown in the screenshot has nothing to do with the Next inspection Date other than to inform the user of the last inspection to help the user determine if the next inspection should be the same time span or if it can be different in the future I hope this is useful.

Truly perplexing. To help, more information is needed. Created in Connect, I assume the form is available from the site but individual questions seem to vanish intermittently? Does this mean once the question vanishes it will return on refresh but will consistently be gone in future openings of the form? What version of Connect is used to create the form? Is this form being opened in the Survey123 app on a mobile device or opened on the Survey123 website? If a Survey123 app, which version is used on the mobile device? What operating system is used on the mobile device? Is this being published via ArcGIS Online organizational account or via your Enterprise Portal? When you view the data on the Survey123 beta website, are all questions presented or are some intermittently vanishing? If these were answered in the field, are the answers for all questions present? If some questions are missing, do they always return when the form is refreshed? When questions are missing, have you tried creating a Draft report to see if they publish even when missing from the screen? If possible, could you share the XLSForm and your report template to help with troubleshooting the problem.

Referring to item 1 - did you recently update or modify the existing form? And same questions as above. I am stunned that this is still the case. The referenced document by EddyClark was a 2019 document. I appreciate EddyClark for finding it but I sincerely hope someone from ESRI will step in and reference a newer document that updates this information or addresses this inability to create reports that include attachments when working from our own Enterprise server.

I am in agreement with ShawnHarris, we have invested in IPads to run Survey123 in a disconnected environment. I have also experienced the shortcoming's of the webform and now wonder how to proceed. What are we to do after 3.13? Will the webform be upgraded? What will we be able to do in disconnected applications? Do we give away our IPads and switch to Android? Will Android be dropped in 2024? Just getting comfortable and it all changes again.....

A song comes to mind but it sure dates me... "Another day older and deeper in debt..." but it could be in this case Another week older and nothing has changed... I once again published reports from the Survey123 Beta Website and once again the reports all have the same date. I need to open each one to get the correct date then change the name of the report to the correct date. I publish both as .DOC and as .PDF with the same results. I have opened the form in Connect and verified the field name for the reported date is correct. Could it be a problem with the default value set as "today()"? Is there another option for getting today's date as the default"? This is working since each report does have the correct recorded date but is this causing a problem in the reporting function on the beta website? Just tossing out ideas. Still awaiting work-around or suggestions. Any one else having a similar problem?

Am I correct in reading the logic to say Not if == 1 or if Null? This implies any other answer would trigger the actions. You could try ${af_cleanliness_issues} != '1' alone. If that works then add the or ${af_cleanliness_issues} ='' with parens as above. Single Quotes around 1 might be tried also.
