Hi Alex, Currently, the answer is dependent on a few things. We don't support connecting to a Feature Service for lookup yet in the Survey123 field app or the website. Using Survey123Connect, you could author a survey that has a select_one with the listing of the addresses; refer to https://community.esri.com/thread/186982-1k-5k10k-pushing-the-limit-on-external-choices . That could then be used in a pulldata function to lookup additional information in an accompanying CSV file. Currently, that survey would only be usable in the Survey123 mobile app, not the website. Working with information from existing feature services is an enhancement we want to be able to support.

Currently the person sending in the additional information would need to use another application (like Collector, an editing web application, Web Appbuilder, Pro, etc.). We are working on including update capability into Survey123 in future releases.

If you're populating from a select_one, the values need to be in the 'choices' sheet of the survey. What I think you're wanting to do is have the select_one values populate from a column in the csv file- is that correct? If so, that's an enhancement we are planning to include in Survey123.

Share survey—Survey123 for ArcGIS | ArcGIS describes how to share a survey to everyone once you've authored a survey. There's nothing particularly special about a public survey in the authoring process, though you may want to think about the portion of the public you want/expect to fill in the survey- what is their language fluency breakdown (English, Spanish, Korean, etc.), level of sophistication (e.g., if reporting wildlife sighings, do they know the difference between a raven and a crow). In terms of security - surveys currently published by Survey123 Connect by default are published with full editing controls enabled (add/update/delete), though it takes some technical knowledge to access those capabilities (we don't provide editing controls in the user interface, just data submission). This can be set to add only in the ArcGIS Online/ArcGIS Enterprise settings for the Feature Layer (data table) associated with the survey. Surveys published through he Survey123 website's form designer expose only a 'fieldworker view' of the survey to the public; when published, the default for this is add data only.

Ah, thanks, With Survey123, you can use a PC to take surveys in a standalone environment, but the data storage is still tied to either ArcGIS Online or an ArcGIS Enterprise organization; either is required to publish a survey. It is possible to install ArcGIS Enterprise on a single PC (I've done that routinely for demonstration purposes), but if you're only using it for Survey123, you aren't getting a lot of value out of it- Survey123 is designed to complement a data/mapping system, not be an independent application.

Hi Omur, At this time, the solutions described above are the best solutions for printing out the surveys. We are interested in supporting printing of the survey data with a custom format, though we don't have any immediate plans to focus on it. Additionally, this sort of functionality is something that could benefit the entire ArcGIS platform and we would a solution that can support the requirements of multiple applications.

Hi John, Survey123 is designed to work in either a cloud or on-premise enterprise environment; it requires either ArcGIS Online or an ArcGIS Enterprise organization to store the data. Given you're other question on public surveys (https://community.esri.com/message/659893-survey123-public?et=watches.email.thread ), I'm a little confused- a public survey would need to be accessible over the network and thus a standalone PC would not be a great deployment target.

That's interesting - behind the scenes we're doing the exact same thing ArcGIS Online does to make the fGDB copy, we just then automatically download it and then delete the fGDB item so it doesn't clutter your items.
