As far I as know there is not solution to this problem. As the cost of high accuracy positioning comes down ESRI are going to need to address this issue. We have a workaround in our workflow but you are essentially describing where we are at as well. We are currently developing a local solution using python scripting and a geoid model (for us in Canada we use a height transformation model). We do have a workaround using Trimble GPS Pathfinder office but this causes problems with the output shapefiles (field lengths and shapefile naming constraints) and you can't utilize geodatabase checkout and checkin of features. I agree that ESRI needs to step on board with a truely 3D coordinate transformation. We always survey infrastructure to cm level utilizing RTK but have the problem of trnasforming the collected ellipsoidal heights to orthometric. Not a problem for points, but for lines and polygons is a major pain. We have a workaround but this fragments our workflows immensely. All we need is the option to input a geoid model into Arcpad to do this computation on the fly. Seems like every other mobile survey software can do this, just not in the context of GIS collection utilizing attributes as well as geometry.

Jeremy, I have ran into the same issue and I use XTools Pro to extract features to shapefile/geodatabase, then export to CAD using a seed file. In Xtools Pro use Extract Map and you can define what to extract (current extent, selected graphics, or selected features in layer), just define output shapefiles or geodatabase and thats it. Then run the export to cad and you have your area of interest or features of interest into CAD. Hope this helps, Shawn
