Hi @CrisjovanoGumaraes , The Likert appearance is behaving as we expect - Likert questions published from Survey123 Connect display in the web app the same way as Likert questions published in the Survey123 web designer. Is the concern that Likert questions are always horizontal in the field app?

Hi @DeonLengton , Are you able to export from the Item Details page of the feature service? Survey123 uses ArcGIS Online's Export data capability; if export works there but not in the Survey123 website, then there is an issue that needs to be addressed in the website.

Hi @JacobRothberger , There is a typo in the choice_filter in the XLSX file you provided - 'Region' is capitalized when it should be 'region'. Once that is fixed, the form works as expected when I publish. That being said, that isn't the error you are reporting. Just to check, is your ArcGIS Enterprise using IWA? If so, please check that you published with the latest version (3.13) or have version locking turned off - what you are describing sounds like BUG-000126788, which was addressed with 3.13.

Hi @DarraghOSullivan , Regarding the related records being limited to a total of 1000, this is likely due to the maximum number of records a feature service provides in 1 request. This can be modified to increase the number of features; additionally, BUG-000138707 has been filed to have Survey123 paginate (request the next set of records).

Hi @Anonymous User , Can you provide details on how the names were placed in the XLSX file? One thing that occurs to me is if they were copied from a rich text source (say, MS Word or copied from an HTML page), they may have HTML embedded that wasn't visible. The HTML appears to come from a WPForms, which is a WordPress Forms plug-in (note the wpforms value in the name attribute). You can alter the alias in the feature service's Item Details page - go to 'Data' and select the 'Fields' view. Click on the field you want to re-name and edit the name:

Hi @Anonymous User , This is to be expected for the field aliases - your form labels have HTML and they are used directly as the field aliases when you publish a survey. You can clean up the aliases using the Item Details page for the feature service.

Hi @OlivierDemars1 , I can see issues with a space separated date. That being said, I don't believe Survey123 explicitly supported a space separated date for parsing dates; the ISO convention of hyphenated dates was what was expected to be supported.
