Hi Emily, There is not a way to remove the 'back' button in a multipage survey. With regard to the thank you notes, is this a form being filled out via the web or the field app? The web app does not have the ability to display different notes based on data in the form; while the field app does not have a thank you note by default, it could have 2 general notes that display depending on a value in the form (presumably, you would have a value in the form to distinguish unqulified/qualified). ... Hi, First, I want to check to see if this problem has resolved "on its on" - there were some issues with the hosted feature services earlier today - see ArcGIS Online Health Dashboard .  The error you are seeing may have been caused by that.
