Hi Elizabeth, What you're describing sounds like an unforeseen interaction between the surveys authored on the web and a change to the hosted services that occurred with the March 1 update to ArcGIS Online. This was corrected on March 9. Any surveys created on the web between then needed to manually expose the fields on the data collection layer- see Enable anonymous submittal - 03/2017 workaround - YouTube for a demonstration.

Hi Charmel, The best way to do this currently would be to have the disclaimer presented as a note with a select_multiple or select_one question to provide the acknowledgement. Can you describe the target audience a bit? Is this for a crowdsourcing-type application (as in a workforce this should be covered in other rules and regulations)?

Hi Luca, Survey123 comes with the 'Builder' level of a developer subscription (see Pricing | ArcGIS for Developers - it's included with the 'ArcGIS Online Organization' component). Again, if this is for noncommercial purposes, the ArcGIS for Personal Use Program | ArcGIS for Desktop Advanced for Personal Use would be more appropriate.

Hi Karen, Which application(s) did you use to scan the encrypted QR code? In doing a bit of research, the only application that does this is the Android app QR Droid, which is doing as a feature of the app- there isn't any standard for encrypted QR codes that I can find. (Secure / Encrypted QR Codes - Stack Overflow and QR Droid Mobi are what I could come up with).

Hi Omur, Given where you're at, submittal from your computer or extraction is the best option. If you've copied the images over along with the sqlite databases, place them in the 'My Survey Attachments' folder to have submit properly. Alternatively, I have a python script that can read the sqlite database and extract the data- let me know if you want to pursue that route.

Hi Frieda, With regards to security of form information: On Mobile Devices: Survey123 currently does not do any encryption of it's own and relies on the encryption supplied by the phone. In general, modern iOS (version 3+) and Android (version 6+) phones encrypt the file systems (all files) of the device when 'at rest', i.e. when powered off. We have an enhancement request to support encryption, but it has not yet appeared as a high user priority. On Server Storage: Survey123 data can be sent to either an ArcGIS Online organization or ArcGIS Enterprise (on premises) implementation. For additional security information about both, please refer to http://trust.arcgis.com - ArcGIS Online: Data access control is secured via ArcGIS online's authentication. Data is not encrypted at rest. - ArcGIS Enterprise: Hosted Feature Services (default table) do not encrypt data at rest; Survey123 can, via a ArcGIS Server Service, write to an enterprise database, whose encryption status is controlled by the System Administrator and Database Administrator. In addition to your questions of on device and on server, let me also address data in transit (from mobile application to server) - if the destination server supports SSL, the data transmission is encrypted and sent via SSL. Survey123 is available as a template in AppStudio; a custom version can be modified to support encryption of the data or to delete a survey once successfully submitted.

Hi Luca, Survey123 requires at least the author of the survey to be a named user in an organizational account with ArcGIS Online (or the on premise equivalent, ArcGIS Enterprise). There is no storage limitations; however storage does use credits when using ArcGIS Online (photos are stored at 1.2 credit per 1GB per month). If you don't mind, may I ask about your survey? If this is for educational or other noncommercial use, it might be eligible for a reduced-cost program.
