Looks like I had two locations within the report template that was causing my report failure. Once I removed the second ${ScienceName} from my report it was able to run fine. Good catch. Thanks!

I am receiving similar error Error occurred when checking the report template. Failed to parse "${ReportDate|format: ’MM/DD/YYYY’}"Field "${ReportDate}" does not exist or cannot be found in current parsing scope. However mine says the ${ScienceName} field is missing. Which this report worked earlier in the day on friday and now it wont work. Did you find anything on this?

Has anyone created a nice report for Survey 123 but have issues with field name lengths and causes data to not appear in locations you want them to? Has anyone seen performance issues when using SQL Server Esri Enterprise Geodatabase with data that have M values. We are running across slow draw times on all of our GIS datasets that were derived from our LRS network GIS layer. The same datasets will draw within 1 second in a file geodatabase vs in sql will take 10-15 seconds. Thanks, Claire

We changed the location of the default project location and that fixed the issue. Looks like something weird regarding permissions in the default folder.

Basic License - Named User. Another user with same configuration except for Windows 10, as similar issues but can save it if changed location of project. But this one is Windows 8.1. I am having the user test out if they change the default location of the project could be permissions within that user profile.
