31 is the limit for SQL Lite which is what Field Maps uses to go offline so that makes sense. GDB is 64. I wonder if the new offline checker looks for this?

The reason this is not firing is that they are not editing the parent. The form of the parent never opens so its not going to run. A attribute rule may do it but those are not in AGOL yet. I do not think I have seen a way to edit a layer you are not in at the time which makes sense. Two older school options I have used. Run a python script on a schedule to check for new inspections and then update the parent field. Or what I like to do is just make the related table a related feature class. Then you can just map the inspections themselves. This is also handy for times when crews mislabel the inspections as you can clearly see where they actually were standing. Many benefits to always grabbing spatial info, no real advantage to having it be a table. But if you already deployed this as a table it would take some work to change that. Hope that helps

Sounds like you made the service then connected the form to it? You list uppercase field names but the fields 123 adds are actually lowercase globalid and parentglobalid. I have seen this be an issue but I have also seen that of you try and change this in the backend Arc thinks they are the same. As a test I would create a brand new form and use the file option to copy your current form. Take out any submission url if there is one. Publish this as a new form. Look at the schema it makes and compare. Then add a submisson_url in this new form pointing to your current feature service and see if it gives an error. That will give you some more clues. You should be able to fix this in ArcPro. Use the 123 relates and it will allow you to figure them out. Select a parent, follow the relate, field calc in the site_id, repeat. Will be a bit tedious depending on how many records. You could script if you are capable of that but easy to get into trouble with that also. I suggest export to a GDB from AGOL as a backup first. Then you can edit the service directly in Pro.

No you just add a field inside the repeat called say SiteIDRepeatName then give that field a calculation of ${site_id} to pull the value into the repeat. You are just trying to get the site_id value into each subtable.
