It's possible that something went wrong with the configuration of that original survey. Anyway, I am glad that you were able to resolve the issue by re-creating it. I am still curious if I can reproduce the same issue with that survey though. To upload the file, click on 'Use Advanced editor' option > and there will be 'Attach' button at the bottom right corner. Glen, The team is aware of this issue and they are working on it as mentioned here: https://github.com/Esri/Survey123Community/issues/278 Here is a workaround option: Sign in to ArcGIS.com Click on My Content Find the Survey folder from the left and click on it Check the box next to both items (code sample and feature layer) Click Share and select the group you want to share this survey with. Brian, This might be a network related issue. When you click the Sign In button is Survey123 Connect, the following requests are sent to ArcGIS Online: https://www.arcgis.com/sharing/rest/oauth2/authorize/?client_id=survey123connect&grant_type=code&response_type=code&expi… and https://www.arcgis.com/sharing/oauth2/apps/survey123connect/vendorinfo/thumbnail.jpg If you click on the first link, do you get any response? Copy/paste the second URL in a browser and if you get any response. The first one should return a sign in dialog page and the second an Esri logo. If you don't get any response, that means your firewall might be blocking it an you will need to work with your IT and whitelist the ArcGIS Online domains listed below. As you indicated, you can sign in fine on Survey123 App on a moble device. By any chance your mobile device is on a different network? Domains to whitelist/domain exceptions: http://*.arcgis.com https://*.arcgis.com Provides access to the main ArcGIS Online site and capabilities http://*.arcgisonline.com https://*.arcgisonline.com Provides access to basemap tiles https://ago-item-storage.s3.amazonaws.com Used when items are downloaded from ArcGIS Online. Craig, Let's try this method then: 1. Upload your tile package to ArcGIS Online My Content (uncheck the option to publish) 2. Copy the URL (For example: http://ess.maps.arcgis.com/home/item.html?id=ec54ab5ca97a441d9482292694989101 ) 3. Open the .info file and delete everything and paste your code here. Make sure that you add the URL of your own tile package here. The code should look like this: 4. Save the file and publish your service. 5. On the iPad, downlaod your survey, and open it. 6. Click the menu and Download Maps 7. You will see your tile package listed here for download. 8 When you click Collect, the map will be loaded for you. 9. Just for your reference. Craig, I'm not sure if you saw my other response above. If you already have the Tile Package loaded on the device for offline use, you don't have to make any changes on the .info file. Just publish your survey > download the survey on your device > click Collect > click on the map section > click on the menu on the top right corner > and select your tile package. Please follow the steps mentioned this blog (Prepare basemaps for offline use ) to copy your tile package to the device. Fasil

Craig, Thanks Hannah for the tip. I used http://jsonlint.com/ to validate the code snippet, and found out that there was an issue.
