Looking at the Topic Report, it appears this template is already set to Custom from the start (when you create the app). Which menas this template is customized by default to provide a different experience on mobile, rather than Auto syncing with the Desktop layout. You may need to switch back to Auto and then back to Custom to pull in your changes from desktop. But when doing this, you will lose any existing mobile customizations that were made, as the layout will sync back with what's in desktop.   ... View more

This will be addressed with the June release of ArcGIS Online. The feature title displayed in the Edit widget will come from the popup Title you configured for the layer in Map Viewer, so you'll be able to change it there. This should also support expressions! ... View more

Adding the idea from @Shen_Zhang above: have you tried placing the edit widget into a widget controller? You can also configure a message action to open the Edit widget within the controller automatically when a feature is selected from the table or a button is clicked to create a new feature.
