Hi Nick Moore, This could happen to earlier surveys created in ArcGIS Online or all surveys created in Portal for ArcGIS. In details, Survey123 is using a feature called "keywords" property of attachment of hosted feature service to store the question name ("name" column in XLSForm) for each attachment, so that we can distinguish which question each photo belongs to. This feature is available since Dec. 2016 release in ArcGIS Online and we finish the implementation in Survey123 by Mar. 2017. So we cannot tell which question a photo belongs to for surveys published before Mar. 2017, or surveys in Portal for ArcGIS due to the feature is not available in portal. Basically, you can tell from the Individual Response view in Data page if your behind feature service supports the "keywords" property: If you see photos appear under corresponding questions, then "keywords" property is supported by the underlying feature service If you see photos appear in a single "Attachments" node, then "keywords" property is not supported by the underlying feature service Is this the case for your survey? If not, can you please send us the XLSForms file and the edited custom print template file (.docx) for investigating? Thanks,

Hi Naomi, Seems you've tried on creating the survey from scratch in the web designer but still encounter the problem, right? Would you mind sharing the URL of the "borken" survey you've created with us, such as https://survey123.arcgis.com/share/{survey id}? It will be very helpful for us to investigate the issue. Hi Amy, Thanks for the feedback. This is currently a known limitation on our radar. When creating a Dropdown or Single Choice question in web designer, coded value domain will be created in the behind feature service. To simplify the user experience, the choice item label input by users will be used as the names while auto-generated choice0, choice1, choice2... will be used as values. In the meantime, we're using the ArcGIS Online export operation behind the Download feature, only values of coded value domain will be used if you choose CSV format. Workarounds: Using Survey123 Connect to control the underlying value of a coded value domain Download as File Geodatabase first, Problem: Exporting a feature service with domains to a CSV file shows coded values instead of descriptions How To: Export a feature class to a shapefile that contains domain descriptions instead of domain codes

To help us better shooting the bug, can you please share some steps for how the survey is created from a non-federated feature service? Or it just follows the exact steps described in How To: Create Survey123 for ArcGIS surveys that connect to non-federated ArcGIS Server services ?

If you're using one of the default Esri basemaps, such as "Streets", "Topo" etc., it's a known issue that the home location will miss when you open the survey in the field app. Will let you know once it's fixed.

If you're using a custom basemap in the GeoPoint question, When opening the survey created from the website in the Survey123 filed app, home location set in the web designer will not be supported in the Survey123 field app until we implement the support of a web map in the Survey123 field app.

Due to a web map of GeoPoint question set in the web designer is not supported in the Survey123 field app for now, to apply same home location (zoom level+map center) to a different map (the default one in Survey123 field app) is usually not helpful. We will let you know once it has been fixed.

Is that possible to share the survey to Everyone then post the survey URL here? So that we can investigate if there's something wrong with the particualr survey.

Thanks Sclarke-esristaff for sharing the helpful experience. However, it might not be able to fix all cases. It is currently a known issue that the Collaborate page is not working properly if the survey is created from an existing feature service in Survey123 Connect. Will post here again once we have fixed the issue.

Hi Michael Jabot What you saw in step 6 is expected, the "Members of the Organization" will always be checked if you check the "Everyone (public)" due to any members in your org can definitely access the survey if it's shared to public. Is there any other exception you've encountered until step 9?
