@Kathleen Brenkert : I'm having the same issue. Do you remember if and how you resolved this? My resource proxy is using the same config as yours. I'm getting the same exact 499 error code with "Token Required".

Totally missed the point of single sign-on. Our IDP allows us to provide a URL which will by-pass this screen, but we don't know what that URL is for portal. There must be a URL /tokens/signin or something. We tried the /authorize and /signin, but no luck. Anyone know of the URL i'm talking about?

Do you know if Enterprise GIS environment federated to portal can be of mixed OS? Or Is it recommended that all servers portal, arcgis server, web adaptor be of the same OS?

Okay, false alarm, my problem came back. I was told by ESRI that it is a bug. They have a patch for 10.4.1 and it will be fixed in 10.5.1. I upgraded to 10.4.1 and applied the patch.

I don't have a link. They just gave me that bug information and description. I did seem to have resolved my problem. I simply republished my service and all is okay now. I can add the attachments and I no more of those errors are logged. Good Luck!

I contacted ESRI premium support about this. They said they knew of it, it's a bug and has been fixed in 10.5.1: BUG-000100553: Receiving "An open transaction was detected. All user transactions must be closed prior to StartEditing" error in the ArcGIS Server logs when applying online edits with a non-versioned feature service What I don't understand is this was working fine before and all of a sudden stopped. There must be a work around to resolve this like republishing the service, re-installing server, something because it's not an issue out of the box.

I was able to resolve this by having either of the following set in feature access capabilities: only the SYNC and QUERY capabilities. OR all the capabilities (CREATE, DELETE, QUERY, SYNC, and UPDATE).
