I had the same issue, but could fix by republishing it. What I did was creating new survey, and copied the content in the form into new form. I am still not sure what was the issue- everything else was the same.

Hello all, I created a survey with an existing feature service (address points) for building damage assessment. I understood this would be 'geopoint' in my survey, and the survey would not create any new geometry when I am using Inbox to attach the information on these existing points. But it seems I have not fully understood yet. As shown below, the existing points are located on the map (as expected). If I click any record, the address point shows nothing, just 0, 0 lat and long in the summary part (right pane). I am not clear why it does not pick up the actual location as displayed in the map area. Currently my survey questions are added on the feature service layer, and I have a related table published to accommodate pictures and audios only. If the survey still try to pick up new geopoint location in spite of having data from existing points, should I make all of my survey questions in the related table and make geopoint auto populated based on the existing point's location? so basically create a new dataset with geometry? Any advice?

Hello James, So I cannot create the relationship with the existing feature layer because the new table would be published under different service...? but would it be work if I download the data from ArcGIS online,and overwrite the service? 1. download address feature layer/related table (table A - being used for collector) from ArcGIS online. 2. Create a new table (table b) that will be used with survey123. 3. Create a relationship class between the new table (table b) and address points feature class. 4. Two relationship exist within address feature points as a result. 5. Publish 6. Create survey with the address feature layer, but delete begin/end repeat for the collect table(table A) so that no data is captured under collector table.

Hello, I am wondering if the scenario below would be feasible. There is a collect app created with address points and its related table (let's say this is table 1). All collected information seems to be stored in the related table only, not in the point feature layer. My supervisor wants to see if I can use this existing feature layer as start point of a new survey123 (using Inbox), and my survey information will be stored into a new related table (table 2). Table 1 and 2 wont have any overlap information and not related except for that they are associated with address points. currently, I cannot republish the data with new additional related table (table 2 - where my survey goes) because I do not have the original data as well as the collector is in use - I cannot overwrite or start as new otherwise the captured information would be useless. So here is my question.... Can I publish a table (table 2) that will hold my survey information, and create relationship with this existing address points layer on ArcGIS Online instead of publishing the whole new set of layers? Then use it to create my survey? I am wondering the existing related table (table 1- used by collector) will be detected as its another related table, and create repeat in my survey form? but I need to delete.. cannot use... Hello Mike, Thank you for sharing this information. I was also looking for any workaround for the case that requires to update domain coded values frequently - possibly daily basis during emergency season. Has there any better methodology come out yet since 2018..?

Hello Jose, I tested my survey in the web browser as well and this happened. I have not tested my it in mobiles as it is supposed to be used in the office environment. This maybe another limit of surveys in web form?? It looks like there are a number of functions that are not working yet in web form versions.

Hello Ismael, I did more testing on my issues because I do not prefer using additional group unnecessarily. However, it looks like having 'minimal' or even 'compact' does not prevent from populating empty record in every related table when I am using conditions in 'Relevant'. Is it an expected behavior? I am wondering I could not add repeat because my feature layer is already published without related tables defined? I will check Brett's suggestion, too. Hello Brett, Could you please provide more information on the alternative way? I published feature layer without any related tables (except for the attachments). Should I add relational table in the original data and then republish it? Hello all, Is there any way that I can add spatial control for Inbox? For example, my survey is built on the address points in the province which has more than 1M count. I've already set up SQL query in the survey so it only filters where surveyStatus = null (then surveyors need to go check), but there will still be too many records. I hope I can set up spatial query so that Inbox display address points that are within 100 m (or any specified distance) of surveyor's location. Is this functionality feasible in survey 123? If not, are there any limit on the records that can be in Inbox?

Hello all, I have created a survey with an existing feature layer. The survey is supposed to capture a few images and 1 audio, but when I tested it on the phone, there are no icons where I can activate my cameras and voice record. There are just names listed, Image1, Image2, ... Image 5, Audio. Before publishing the feature layer, I ran the tool 'Enable Attachments' and there was no error when I published survey. Any advice where I need to look into?? Cheers, + Image/audio seems work when I test in in the survey123 connect preview, but not in the downloaded mobile app. + Additional Note My current method is selecting the existing feature from Inbox> open survey. This case I do not see any image/audio buttons. However, if I start with 'Collect', I can see them and can capture images/audio. So it looks like I need to enable to add/edit attachments of the existing features.....any advice?
