Are you able to provide a sample of the data you're working with? If so, we can test here and see if it's the data or something within your installation that has become corrupt. I assume it's not the data, since it's been working, and you've tried different data.

What about using a reference scale on the data frame? Go to Data Frame Properties > General > Reference Scale. Set this at 500, the same scale that you scaled your points at.

Yeah, it definitely should be recursive through your subfolders. I assume that since you can register each individual folder, the permissions should be okay. Is the data on your local system? Are you using a drive letter or UNC path?

This should probably go into the ArcGIS Online section, but unfortunately, if you're loading the data directly into the web map, you're unable to edit the data. If you don't have many points, you can look into adding them manually, but that of course may not be an option if you're adding a bunch of points or the attribute table contains a lot of information. Are you on a personal account, or is it an organization?

I've seen this once before; copy and paste the contents of the mxd to a new one. In layout mode, you can drag a box around your whole data frame and then copy it into a new mxd.

You can Dissolve on the city name or whatever value identifies where the points are located in and then use the Statistics section to get the Count of the field you're dissolving on.

Are you using a feature class as the extent to cache? What update mode are you using? Have you tried to only cache those specific scales, one job at a time? Also, it looks like you're using 10.1; do you have SP1 installed?

What type of data is it? File based, such as feature class or shapefile, or SDE data? If you're using file based data, is the data on the machine where ArcGIS Server is installed? If it's SDE, are you connecting with OS Authentication or DB? Take a look at this resource for some information on permissions and data stores.

Matt hinted at it; a managed database means that when publishing, data will be copied to the server, but to an enterprise geodatabase. This is useful if, for example, your ArcGIS Server instance cannot communicate readily with your on-site SDE database. The data will be copied to the ArcGIS Server input folder into an enterprise geodatabase, so that you'll be able to create feature services or WFS services, as they require an enterprise geodatabase. This resource here explains it a little further. Scroll down half the page to the section titled If the service type you want to publish requires an enterprise geodatabase.

Yes, feature services require SDE databases as the location of the data within the service. The help lists the steps to go through when you want to publish a feature service.
