Hi Colleen, To asnwer your questions: 1. With image question, only the image format is allowed to upload and if the format is other than the supported image formats, it will throw an error and upload is not allowed. I am not aware of the malware scanning on AGOL. The image question will initially scan the file format and will bock any other image formats which we do not support with. 2. Currently there is an enhancement request submitted for this request. ENH-000116753 Allow disable the upload image from device's files in survey 123 I encourage you to contact Esri Support. Our Support team will assign an official enhancement number for your records. Similar requests from other customers can then be attached to the same enhancement request, which helps us assess demand for the enhancement and prioritize it accordingly.

Hi Elizabeth, Could you check the following items to identify where the problem occurred? 1. Go to the org and open the main feature service with webmap viewer. Check the name in the attribute table. Does it show Daniel or Don? 2. Now open the stakeholder view layer in webmap viewer, does it show Daniel or Don? 3. If step 1 and step 2 showed different name, you might have the sync issue with the view and the main layer and the above article should resolve it when you unshare and reshare the survey. 4. Now check the dashboard and see if you can manually point the layer to the stakeholder view by click that "Change" button in Data options. Make sure step 2 shows the correct data first. If you are still encountering the data sync issue, I would suggest you to contact Esri Tech Support and open a ticket to have the Tech Analyst to help you troubleshoot it further. Thanks,

Hi Patrick, It sounds like you are looking for the "Cluster Points" function on the webmap viewer. You can enable the Cluster Points with your feature service on the webmap viewer and you should be able to see the representation you want.

In your xls form, I can see you are using quite a lot of hidden fields, caluation, and pulldata funtions, which might potentially impact the behavior you are seeing in the Inbox. The image question is not availalbe in Inbox due to this enhancement request: ENH-000110614 Add the ability to allow additional attachments to be added when attempting to edit and resend surveys in the Survey123 Field App I'd suggest you to contact Esri Support to have them help you troubleshoot the issues individually and also attach your case to the existing enhancement request of the Inbox images.
