You have several name issues. First Date is a reserved word. See the green mark here telling you. Here you have a space after the repeat name. Again cannot ignore the green boxes on the name column. For your species list just an idea but what we do is have species code scientific name common name with spaces in the label section so crews can look up any of the names they know. Start with these fixes and see. Hope that works.

Also seeing that the CSV is getting the field alias, which we just added spaces to. Most software we feed the CSV into cannot have spaces in the field names. So we are stuck there. No way to configure this. https://community.esri.com/t5/arcgis-experience-builder-ideas/table-widget-export-to-csv-is-using-the-field/idc-p/1658539#M4623 For poster you could create a view and then use that. Total pain but if you are stuck it should work.

Bump we are struggling to have users export a working CSV still. We made our own custom GP tool for this but we have other bugs that make this tough. Bump on this at 4 months. I have had multiple Esri staff agree this is a big issue but can never get anyone on the ExB team to respond. We have sent videos but can resend. We are stuck without being able to use our custom GP tools. Sure seems like a bug. Can some one please take a look at this. @AlixVezina

You several fields using the same field name Species. It should be flagging this with a green mark in the cell. Cannot dup field names.
