Do your point features have M values? You can check by the feature class properties and on the General tab under Geometry Properties. If a feature class has M values, it's not possible to edit correctly via a web service.

I believe if you export the feature classes out as an XML workspace document, the related tables will import with the feature class.

I'm looking for a doc that may detail the steps and configuration files that require editing to enable adding custom widgets. There may be some of this in the LocalLayer widget and Rebecca has included it in her blog. Maybe i'm missing something, but some widgets work when added to the ../client/sitemap/widgets folder and others do not. This probably more of my not understanding the design/organization of the WAB and widgets. Any help would be much welcome.

As I've been told, you can not edit lines the at have M values in collector. I had a similar experience where I swear we were able to successfully edit lines with m values at one time and then magically one day it would not work. We have since removed m values from any line feature classes we wish to edit in collector.

Your issues may lie where you are trying to write the output. See the documentation, http://http://resources.arcgis.com/en/help/main/10.2/index.html#/Authoring_geoprocessing_tasks_with_Python_scripts/00570000007r000000/ I believe you need to have the entire folder structure in one location, see example below. , The original script is in the python folder and the script tool is in the toolbox. My input data and schema is in the Data.gdb and directing output to scratch.gdb. Once this is published, it runs as a geoprocessing web service. Below is an example of the output on the ArcGIS Server:
