What type of geographic information is in the CSV? You can only add data using lat/long coordinates or address information.

Open a new web map select Add > A Layer from the Web. Change the drop down on the Add Layer from Web dialog to A CSV File. Paste the URL for the CVS file in box. Click ADD LAYER. Do note that only point data can be added using this method.

Chris, If you are adding an item already on the web ArcGIS Online will not create a REST URL for that item since it is already hosted somewhere...you are essentially using ArcGIS Online to point to the other URL. If you do want a REST URL to be generated for your CSV choose Add Item > From my computer to upload the file to AGOL. Once uploaded go to the item details page, share with everyone, and the the REST URL should appear. Hope this make sense, Kyle

The table doesn't look like coordinates in decimal degrees...maybe they are hidden and if so you want to make sure your data frame is set to WGS 84. Otherwise make sure your data frame is set to match the datum of your x,y values.

Jim, Just covering all bases but have you added your locators/geocoders to the Utility Services section in your ArcGIS Online for Organizations settings? If they are not available here they will not be available in the Search Settings on the Simple Map Viewer app or in Web AppBuilder.

Jim, If you publish your web map as an app with the Simple Map Viewer you do have the option to configure what locators are exposed. I believe several other apps include this functionality as well.
