Adding a checkbox to Field Maps form

I'd like to add a checkbox to field to a Field Maps form so users can pick from a list of options and in turn, the options they pick will get populated in a text field with comma separating their selections. 

Example: 

Filed Name: Hazards
Options to pick from: Steep Slope, Flooded, Utilities, No Parking, Extreme Cold.
In the Filed Maps form, a user checks "Steep Slope" and "Utilities".
I'd like those two selections to populate the Hazards field in the feature class with a comma separating the two options: Steep Slope, Utilities.

Is there a way to add a checkbox field in Field Maps?

I voted up your idea for this, but, since there are other ideas requesting the same thing since at least early 2021, I wouldn't get my hopes up.

Might help to Kudo/comment on the posts, though it doesn't seem to get much traction.

Workarounds appear to be use Survey 123 for this.
