Hi Leila, Could you check the Data>Connection tab under your Power automate account and check the Survey123 Connection status to see if it shows Connected? If you haven't logged in for a while, you might have to re-connect your Connection again.

Hi Dena, If you need multiple location data for your durvey, you can enter 'null' into the bind::esri:fieldType column for others but one of the geopoint questions. There is an sample in Survey123 Connect to show you how you can create surveys with multiple geopoints. We are still working on supporting nested repeat on the web form at this point, hope this would be helful as workaround.

Hi Becky, Could you try to go to our beta version of survey123 website and test to see if the issue still persisted? http://survey123beta.arcgis.com Another way to try is go to Settings>Version and choose "Always use the latest version." option to see if the issue can be repro?

Hi Becky, My initializing form error might be that the associated .csv file is missing. We actually have one issue with the image field on web form. When you capture the image from webcam, it does not fullfill the required validation. So just want to check with you if you used the webcam to capture the image and upload it on the web form? If yes, it's still an valid issue we are working on.

Hi Becky, What version of survey123 field app did you use? Tested with 3.8 beta seems to be no issue with that image field. Could you provide more details of your mobile type and the version of the field app you are having issue with?

Did you try chrome, firefox and Edge and encounter the same issue? What version of the suvey123 Connect you used to publish the survey? And what version of web app did you use to open the survey web form(the Settings page on the survey123 website)? Is it possible you can share the link of the survey?
