Hey John, there is no additional information on the errors? I normally try to narrow down possible causes by trying if you can submit the form from a browser. trying to submit the form only with the required answers. The analysis tool in Survey123 Connect tells me that the template is outdated and recommends an update. I would always recommend to give note questions the type null to avoid the empty columns. The analysis tool also recommends to remove the names from those questions. After fixing these issues I published the form and was able to submit the form from a browser and from the field app but I filled only the required questions randomly as my Swedish is not so good 😉 I hope that helps... ... For me most of the times that error comes when the feature service has a different name than the form_id in the settings sheet of your survey. Make also sure that if you have calculations/questions in your form that are not going into any attribute field to set them to the type null. ... View more

Sorry to hear that the MS link didn't solve your issue. Do you want to try the attached template? It was only downloaded not further manipulated in word. Do you need the remaining questions (answers) to be editable? Otherwise you can just set the inbox to view only. What I normally do if I want one question to be editable (e.g. for a follow up or approval) I publish a second version of the form that references the same layer and put only the editable question in it. Or put all answers as notes and then only "approved" and signature for example editable. I hope that helps. ... View more
