My workflow is pretty simple, 1st. Create the Active Directory user. 2nd.  Add the user to the database through SQL Mgmt Studio. 3rd.  Add the user to the Ent GDB through ArcCatalog. 4th.  Add permissions to either Feature Datasets or Feature Classes. I'm part of an organizational account so things work a little differently.  According to the links ESRI sent you, you should have access to ArcGIS Online Organization Subscription.  Here is an excerpt from one of the links above: Every eligible ArcGIS for Desktop license receives one Named User account.  You’ll be asked to choose whether you want your ArcGIS for Desktop License Entitlements applied to an ArcGIS Online Organization Plan account or Portal for ArcGIS instance.
