Under the Types style you have the ability to group values together. You can name these groups and those will appear in the legend. Most of the time a group of values comes into MV untitled. In this case possibly how the layer was generated was adding this gridcode to the group name. If you open the up one of the layers showing gridcode you will see it has that as a group title. You can click on that and change the name there and see it reflect in the legend.

This is supported in Map Viewer and other 4.x based apps like Instant Apps and Dashboards. You will want to make sure you apply this using arcade when defining it in Pro. This will not work in Map Viewer Classic or Web AppBuilder and you will need to make sure "Make symbols compatible for all apps" is unchecked in the sharing panel in Pro. We just added support for honoring web maps with display filters in Map Viewer. Display filters are currently only authored in Pro but you can set up a scale range and define a filter for that range. I am currently working on a blog actually that will go through this workflow that should be out sometime next week. In Map Viewer it will honor the display filter and you can enable/disable that display filter on the layers panel. The expanding on the layer list behavior group layers is currently as designed but we have sent this feedback over to the team. You will also see this on Experience Builder with an app that allows reordering. You can also use the overflow action and select Move and pick the group or clicking on the drag handle will also give you the option to move to group layers. We still have some performance updates on the roadmap for differences between the two if you can share the map I will take a look.

Not that I can think of tricky part would be number of dashes we use per map scale and width and knowing how the end of a row of features exists and when its just the end of a small feature which in the case above would behave better as a segment of the line.

This is a map image layer and the server is exporting the image which is displayed in the map. Here is the raw export where you can see the similar drawing you mention above. Also the bikeways look to be multiple tiny features I would recommend if you can use the layer at the MapServer/0 URL which will use it as a feature layer and not rely on the export image for drawing and using the vector symbols under Dashed you will get a closer to expected result but still some of the dashes are longer than some of the individual features that are in the map.
