Imported shapefiles or csv's will not work. The Collector app will retrieve maps that have been setup to use the collector under your My Content and to the groups you belong to. It does not search the entire organization for the content. Does your map have an editable feature service in it? Did you refresh your map list to pick up any new maps? I don't see your link in your comment can you repost it? Thanks Russ

If your service is hosted you can do it from your org account by going into the hosted feature service item detail page and click the edit button and down at the bottom of the page you will see the editing capabilities and editor tracking options. If it is coming from an on premise ArcGIS Server you can alter the capabilities in desktop. Russ

You will just need to have an editable feature service with Add/Create enabled. Then the Add feature icon will be present. Make sure the layer is visible. If the layer is off or outside the visible scale range it will not show up for collection. Russ

When you create the feature service and go to the item detail page for that service. Click Edit and then scroll down to the bottom of the page where you can configure the editing capabilities. You should just need to check on the Enable Edits and choose one of the editing configurations and hit save. Then you should just click Add to new map and save that map and it should become a collector map. Russ

Currently there is not a way to do this. The app can only attach images or videos and can only view the pdfs not attach them. I haven't tried this but you could try this.. If you could first convert the pdf to a jpg and view the image in collector, save it to the camera roll, open the image in the photo editor app where the person could sign the form then save that image and attach it to the feature in the Collector app.. Then you can convert that image back to pdf in the back end. Russ

Also you could disable the update capability and just use Create Delete Query which will allow your field crew to create features and move them, delete features and then query them to get the popup info. Russ

Because the feature does not allow geometry update the feature gets locked into the initial position and cannot be updated.If you do not want to collect a feature at the GPS location you can do a long press on the map and on the white callout tap the arrow and on the options that popup tap Collect Here and that will let you collect the point at that defined location that is not your GPS location but you will not be able to update it after it has been created.
