We took a look at the service and noticed that the REST is missing some info under the Types section. It should say inherited for those domain attributes and the app will consume the data model correctly. Without the info in there the service will display just the text boxes. Editor tracking and ownership controls are not fully supported in offline with hosted feature services. You can only use one basemap at a time in  a disconnected map in the Collector. The basemap you choose when downloading the operational data will be the one displayed in the disconnected map. We are working on improving this experience and bringing in the functionality of switching basemaps. You could set up the Esri layer as an operational layer with scale levels at the bottom of the layer control and then have your other basemap ready on device and you can then use the custom basemap to define the extent and download the data. After you restart the app by force closing it through the application manager and relaunch the app try to download a new map and post a screen shot of the Choose a Basemap screen. I would like to see what could be showing up there. Thanks Russ ... View more

Yes the manage section is only for removing and viewing the content that is downloaded/sidloaded content. You can use this basemap you see under manage when you go to download a new map you can choose this WaterUtils basemap and it will be used to help you pick out the extent of data you want to take offline and then it will be associated to that downloaded map. Did you put it into the Collector basemap folder or into the ArcGIS_Collector folder? You will also have to restart the app and when you go to download a new map you will have the option to Choose an existing basemap or you can download a new basemap which will be coming from the web maps basemap.
