Hi All We have some wall mounted monitors with touch screen overlays ... scene viewer works for the page controls and pinch / touch zoom, pan etc doesn't work. I assume (and seeking confirmation) this is expected? If so, when will scene viewer support touch screen? Thanks

Same capabilities. ArcGIS online is updated before the onsite install files are released. Any differences are unrelated to ESRI capabilities; ie where data resides, Intranet only access, access 'behind the scenes' etc etc

We use About extending services—Documentation | ArcGIS for Server with our in-house installation. Unsure if / how it works on AGO. PS there is a bug at 10.4.1 and below with SOIs that causes going offline to fail. This is scheduled to be fixed in 10.5

There are probably a few options ... the other one you mentioned is to have a related table for each inspection. You could also have an attribute(s) for each year's recording. 'Best' option would depend on what analysis / reporting you wanted to do.
