@rsun_TQB at this time, this is by design, Field Maps web app doesn't support changes to non-hosted feature layers that involve either adding new fields or updating the default value for Feature Templates. To clarify @JanetSilb_Spike ; assuming you're using Online, you can open the web map associated in Map Viewer Classic as it was updated to maintain the form information.  If you're on Enterprise, this may differ depending on what version of Enterprise you are on. The only caveat is to not make updates to the web map in Map Viewer and Field Maps web app at the same time; i.e. in 2 different tabs or browsers, that will cause issues and potential data loss. @Rich  sounds like you should be able to remove the Templates; you own the layer.  Can you check if you can add a new Form Element on the Forms tab?  That might help narrow it down. Another question - are you trying to delete the last Template in your layer? Craig ... View more
