Adding a Model as a Button in ArcMap is not Saved in all Map Documents

I am using Desktop 10.1.  I am able to easily add a model that I created in Model Builder as a button on any toolbar in Arcmap.  However, that button is only appearing in the mxd that I added it in, not in every mxd that I open.  When I add other tools in customize mode they are available in any mxd.

I suggest you check the option to save all customizations to the map document as described here: ArcGIS Help (10.2, 10.2.1, and 10.2.2)

Haven't tried it, but did you try saving it to your default *.mxd?  This won't solve the problem for existing projects but it may for future ones.

Excuse my n00bidity, but where would a default *.mxd be found??
