Surveys look empty when viewing from inbox or sent box

Have any of you experienced this? No matter which records I tried to view or edit from the sent or inbox, the survey appears empty?
I have not set a where clause so I am not sure where could this coming from
Any ideas?

Would you be able to share your XLSFrom and any related files in the media folder for testing on our end?
