New survey form on existed layer and restore deleted form

Hello,

I have two question. 

1. I  accidentally deleted survey form from folder. And layer remained. Is there any chance to restore deleted survey form(not created by XLSform)?

2. I tried to create new survey form on existed layer. But have not found any solution. Could you all assist me?

 Not to my knowledge. The only way to create a survey against an existing service is to use Survey123 Connect.
