I have an image in my media folder that I am referencing in the media::image column. The image shows on the desktop version but after publishing the survey, the image does not display on the web version. What am I doing wrong?

In the media::image column you don't need to enter the full file directory for the image, just the name (with extension) of the image (case sensitive) as it is in the Media folder.
ex.
CLSS_Logo.jpeg
The Survey123 website is not able to access C:/users/ABCD/ArcGIS/My Survey Designs.... as that is local to your machine.
