I made sure that it was shared with the agency and the group in my Content as answered by James T. and that didn't help.

Hi Debra,
Are you using Enterprised portal or ArcGIS online? One thing you may want to try is delete the stakeholder view(back it up if needed) and unshare/reshare the survey to see if you can save the change. If you are still having issue, you might need to go Survey123 websiet>Survey>Settings>Versions>choose "Always use the latest version" and would probably need to republish the survey again to fix some internal issue.
We are using AGOL. I deleted the stakeholder. I think that worked. Having my customer try it again.
