None of my users can access surveys via the website this morning. There have also been a lot of random time outs and generally poor performance with the website over the last week or 2. Is Esri aware of these issues?

Are you still experiencing issues accessing surveys or the survey123 website? I do believe there were some known issues with AGO earlier this week with different AGO tenancies, however it appears those issues have been resolved and working fine now.
If you are still having issues I would suggest you raise a support case with your local Esri support contact and have it escalated to the AGO team.
