Is it possible to add an info button next to multiple choice options to further explain what the choices are? For example, include FEMA's description for Minor, Major, Affected, etc. next to each answer choice for a damage assessment survey.
We do not current support tool-tip pop-ups in the survey form or have an open enhancement request to do so in the near future. Can I suggest you raise it on theArcGIS Ideas page for Survey123 so others can vote on it or alternatively raise it via Esri Support as a new enhancement.
