QuickCapture buttons not updating

I'm creating a QuickCapture project for a 6th-grade classroom.  We will be collecting data about trash.  I've created multiple QuickCapture apps before and can't figure out why the symbology won't carry over from the layer to the project.  Here are my steps.  Attached are two images.

• Created a point feature layer
• Added the field, Categories, and a List of Values (food, cans, glass, paper, etc...)
• Set the symbology within the visualization view (new map viewer) 
• Opened QuickCapture and created a new project using the existing layer just created.
• I established a list of 10 values., but the only button that pops up is New Feature.
