The Use Suggestions checkbox is disabled for my locator even though I built it with suggestions enabled. Is this an intentional limit of functionality or a bug?

In ArcGIS Pro it is possible to use suggestions for custom address locators you have created with suggestions enabled as long as you publish them as a geocode service, then add the service into to the project. Local locators currently do not support suggestions in Pro, only services. There are a few other ways to accomplish this depending on what applications and environment you have.

What do you mean by a custom locator? Does this mean you have changed the default ESRI style?
No, sorry, it's just a locator I built with the Create Address Locator tool, it derives from the US Address - Single House style.
Is your first screenshot where the Use Suggestions checkbox is disabled from AGOL with the geocode service? Or are you accessing that screen from ArcMap or ArcCatalog?
What version of GIS software are you using?
Does this link provide some reasoning as to why this is not working for you?
