addattachment returning 500 error on all services

using javascript api 3.14, i am getting

<b>Error: </b>Error parsing multi-part request<br/>

<b>Code: </b>500<br/><br/>

on all my feature services that have attachments enabled.  These services previously worked with earlier versions of the api, so I am not sure when it broke.

Any ideas? I can add attachments fine in desktop.

Data is in sde, versioned.

ok. We have to put something in place this week I'll let you know what i find

I reconfigured a second time, and it seems to be holding the settings this time and I am able to add attachments.
