Credential Required on Submit

After grabbing a GPS point location, populating its attributes and hitting submit, I am getting a Credential Required pop-up, stating "You need to sign in to access '(null)', and asks for Username and Password. I am already signed into AGOL and using the Field Maps app. What else do I need to sign into?

Is there any documented solution to this? Having this issue at the moment. Please for the future, if the issue has been resolved please document the solution. Thank you!
