how to get time stamp on date?

I am trying to record the time as well as the date when recording a feature in the field on collector.  Can anyone help me understand how to do this?  I was able to do this with one service that I published but not any since.  I have followed the same procedure but nothing has worked.

In the popup select a format the displays the date and time. This should give the option to pick the mm/dd/yy 12:00 AM in the app.
