Shortlist Template that allows two website links

Can anyone point me to a shortlist template which allows me to enter two websites?  I'd like to have one for the feature and another to mapquest directions.  Thanks.

The Shortlist created by this author has been customized to show an additional hyperlink field in the Details panel:
