How to publish the geoprocessing tool "Table to Excel tool"

Hi all,

I am wondering if you all have published the tool(Table to Excel) on your web application. I want to create a web application and so users can download the data within a selected box. The attached image shows I used this tool in ArcMap environment. Could you please tell me if I could have a similar window in my web application?Therefore,  users can select the input table, the location of output folder and the format of file either in CSV or XLS.
