You need an editable layer so the map gets flagged to be opened by the Collector. You can also disable editing on a layer in the web map and the app will respect it but you will still need at least one layer editable to get that Collector flag. The items in the map like hosted feature services will need to to be shared along with the web map to that group. If the item is not shared the group it will fail to load for members of that group. Russ ... View more

So the issue is the Temp_ field. It says Temp_ in the attribute list but in the templates area it is showing up as Temp and that is causing the service not to be displayed correctly. If you change the field name to maybe this: Temp_Recorded and republish the service I think the field name and the template should show the same name and the service should work as it should.
