Have you installed the patch for Spatial Analyst (ArcGIS (Desktop, Engine, Server) 10.6.1 Spatial Analyst Patch ). I just tested with a GRID with 0 and negative values and could not reproduce the issue with the latest version of Arc Hydro (/ARCHYDRO/ArcHydro/Setup/10.6/10.6.0.81/ ). For clarification, the values 1,2,4,8,16,32,64 and 128 in the Flow Direction raster indicates that all cells flow into one and only one other cell. When one cell may flow into multiple other cells, the value stored in the flow direction cell is the sum of all potential directions. This usually means that you will end up with sinks - a sink is an area where the flow direction is not precisely determined as going into one and only one direction. When you fill the input dem, you are eliminating the sinks and forcing the water to find a way to flow out. This is the dendritic scenario. The GeoHMS setup should have installed Arc Hydro 10.7.0.19 but it looks like it fails on an incorrect version check for ArcGIS (10.6 instead of 10.7). We will fix the setup. Meanwhile I have posted the 10.7.0.19 setup for Arc Hydro. Christine

The objective of this tool was to setup the DEM to obtain a correct flow direction - this is why it was ok to modify the original values. Note that this tool is now deprecated. You should now use instead the tool Create Drainage Line Structures. It will create a Flow Direction raster FdrStr which ensures that the direction in the stream is the ones associated to the digitized direction of the input lines.
