I have created a survey that includes a single choice field and a multiple choice field. The issue I am having is that when the survey is submitted, it loads the generic field name to the database and not the custom field name. I have attached the file where the data dumps.
The single choice is:
Pending - Lists "choice0"
In Progress
Completed
The Multiple Choice is: (25 Choices)
5. Debris - Lists "choice4"
11. Medical - Lists "choice11"
