Would anyone know if there is a publicly available spec file/site for the ESRI/ArcGIS/Item Description metadata spec? It seems to have various names, likely for the same thing.
It sounds like it is a derivative/inspired by the FGDC CSDGM and ESRI-ISO specs, but with variations from them.

I think you might be looking for https://developers.arcgis.com/rest/users-groups-and-items/item.htm which describe the item. In addition, each item can also have associated JSON data - https://developers.arcgis.com/rest/users-groups-and-items/item-data.htm - which vary between the item types. For example, for items of type web map there is the Web Map Specification - https://developers.arcgis.com/web-map-specification/ - and for web scenes, the Web Scene Specification - https://developers.arcgis.com/web-scene-specification/ .
In general, I think the best place for documentation for any ArcGIS REST API is probably at https://developers.arcgis.com/rest/ .

something more along the lines what defines the XML spec in desktop/pro.
Aha, that metadata - maybe you are looking for https://doc.arcgis.com/en/arcgis-online/manage-data/metadata.htm which says:
* Regardless of the applied style, the metadata is always stored in ArcGIS metadata format.
