Locate Template

Hi All,
Maybe I'm over thinking this but here's what I would like to do.

I need to be able to export a 'parcel report' that shows all of the services (water, sanitary, storm, etc) to a specific property, for locate (call before you dig) purposes.  I would like this report to be able to have user input with customer details, date and any potential comments and of course a map!
Is this possible, or is there something out there like this already?

If someone could point me in the right direction, it would be much appreciated
