Builder does not load correctly

After installing ArcGIS viewer for Silverlight, I launch the viewer as directed in the quickstart guide:  go to Start/Programs/ArcGIS/ArcGIS Viewer for Silverlight 3.1/ArcGIS Viewer for Silverlight 3.1.  The page loads and shows Downloading @ 100%.  I never see the application builder.  Any ideas on what might be causing this?  I am using IIS version 7.  All of the other prerequisites have been met.

Silverlight 5 installed correctly?
Did you install both ArcGISAPI31forSilverlight.exe and ArcGISExtensibility3_1_x86.exe?

Re-start IIS. Clear you browser cache. Re-start machine.
