Which field is causing an Error 500: Creating feature class failed when publishing to Portal?

When I try and publish this survey from Connect 2.8.4 to Portal 10.3 I get Error 500: Creating feature class failed.

The message doesn't tell me which feature class it fails on.

It has nowhere near 500 columns.

We have been able to publish another survey (the Basic Template) without issues.

It has no field names longer than 32 characters and no field names within a repeat longer than 19 characters.

What am I missing?

Hi Roger

Using Survey123 Connect Classic (2.9.3) I was able to publish you form to our Portal successfully, so I dont think there is anything wrong with logic in the xlsx? Unfortunately I haven't got another other thought's other than ruling out form issues

I have now been publishing successfully to our new ArcGIS Enterprise 10.6.1
